薬は殆どが副作用を持つため、一般公衆への利用に際しては通常、厳重に審査する。悲惨な薬害訴訟は長期間に渡る。Most of medicine has side effects in general, hence normally stringent assessment is conducted for public use. Many tragic litigation happened in history taking long period of time for final judgement.
厳重に審査する理由は、リスク許容度だ。The cause of having stringent assessment is risk tolerance.
仮に、疫病の治療薬があるとする。ただ、０．０１％（１万人に一人）の確率で死者が出る。Assume we have a drag to cure damage from epidemic. However there is 0.01%(one out of 10k) mortality rate.
９,９９９人の命が救われ、１人が死ぬ。この薬を認可するべきだろうか。The lives of 9,999 people are saved while 1 die. Should we approve this medicine?
これは、有名なトロッコ問題に似ている。暴走するトロッコが９,９９９人めがけて突進している。あなたは分岐点にいて、トロッコの方向を転轍機レバーで変えることができる。あなたが転轍機のレバーを引けば、トロッコを分岐させ９,９９９人の命は助かる。ところが、分岐先には一人の人がいて、あなたがレバーを引いたことによりその人は死んでしまう。一方、あなたが何もしなければ、９,９９９人の命は失われる。This resembles to famous torolley problem. Torolley, that is out of control, is going to run over 9,999 people on the way up. You are standing just beside a switch lever to change direction of trolley. If you pull the lever, trolley will go another direction and 9,999 people's lives will be saved however, a man standing on the another direction will be run over and killed. On the other hand, if you do nothing, 9,999 people's lives will be lost.
通常の薬は厳重な検査の過程で重篤な副作用の確率がリスク許容度以下の場合に認可する。Normally, medicine is approved under stringent examination process, by checking if fatal side effect rate is below risk tolerance.
しかし、現在、コロナで、あまりにも多くの人が死んでいく局面でどのような対応が良いのだろうか。However, facing outrageous death toll, many may wonder what needs to be done really.
米国で５万人の死者を見ながら、ある人は言う。厳重な審査をしない薬は一般には利用してはいけない。またある人は言う、非常時において、前線に配備する薬は、ある程度の有効性が示されていれば積極的に利用し、死者を減らすべきだ。Seeing US death toll of 50K, one man says no medicine can go public unless approved through stringent examination. Another says, at the time of emergency, if the medicine shows some evidence of effectiveness somewhere else, it should be used at the front line to reduce death toll.
日本ではアビガンという薬が、多くの人の命を救えることがわかってきているのに、正式認定されないため町医者が使うことができず、既に医療崩壊状態だ。病院の受け入れ人数を抑えるため、患者は門前払い状態だ。In Japan, we see that Avigan works in many trials but local clinic still cannot use it since it is not officially approved yet, then this slow action is inviting medical capacity panic. Due to the limitation of hospital capacity, patients are not even accepted to hospitals.
米国では、ヒドロキシクロロキンという薬の有効性が明らかになっているのに、政争の具となり、一般普及が大きく遅れ、５万人を越える死者を出す事態となっている。In US, Hydroxychloroquine's effectiveness is well know in public however, it is politicized and it is not used widely, hence is inviting tragic death toll of 50K.
世界のコロナ対応の現状は直感的には理解しがたい。不意の敵襲（パンデミック）に武器（治療薬）を厳重に検証する時間はない。使えそうな武器（未承認だが実績のある治療薬）で防戦しないと、死亡者が積みあがるのは誰の目にも明らかだ。だが現状は自縄自縛で、不意の敵襲の場合にも、長時間をかけて武器の検証を義務付ける。３か月で勝敗がつく戦争に、９カ月もかけて武器の検証をするのだ。勝敗は悲しいほど明らかだ。Current world's correspondence against Corona is hard to understand. There is no time for stringent check on weapon(cure medicine) upon surprise attack(Pandemic). Unless you fight against enemy by whatever weapon looks effective(cure medicine that looks working but not approved yet), it is obvious to have death toll piling up. However currently world is self-trapping by mandating stringent verification of effectiveness of weapon, taking long time, in the midst of surprise attack by enemy. For the 3-month war, we take 9-month to find out the effectiveness of weapon. It is sadly clear who will lose.
認可された薬がない状態で、多くの死者が出ている場合、現場の医師に為すすべはない。パンデミックの場合、他所で有効であった症例がある薬は、厳重な審査を待たず、患者の了承を得て町医者でも利用できるよう法律体系を変えていくことで、医療崩壊を防ぎ、死者を激減させ、経済活動を維持することができる。今回のコロナを機会に、パンデミックの際に適用する医療法規体系を立法していく必要がある。There is no treatment possible for doctor in general if there is no approved remedies even seeing lots of death by the disease. At the time of pandemic, if effectiveness is shown in other places, without waiting for the stringent examination and approval, the cure medicines should be prescribed upon the consent with patients. Legal framework should be changed to facilitate such workaround as legally legitimate. This should maintain capacity of medical system while also can reduce death toll drastically and keep economic activities. Taking this Corona opportunity, pandemic medical law should be established.
