高齢者へのイベルメクチンの副作用をグーグルで検索してみた

自分の情報収集のため、イベルメクチンの日本の高齢者への副作用についてGoogleSearchで調べてみた。For personal information, I made Google search on the Ivermectin's side effect for Japanese elderlies.
以下自分へのメモ。Below is a memo for myself only.
イベルメクチンの処方添付文書には高齢者は内臓機能が低下しているため要注意とある。Ivermectin's manual warns dosing to elderlies due to generally deteriorated visceral function.
高齢者以外については殆ど副作用は無いようだが、高齢者には若干ハードルが高いらしい。Side effect looks minimal for non-elderlies but looks not negligible for elderlies.
一口に高齢者と言っても、その健康状態には天と地の差が有ることは了解しているが、一般的な高齢者にどのような副作用が出ているのかを知っておくことは重要だ。Catch-all expression 'elderlies' however, may not well explain the extreme diversity of health condition of elderlies. Some are very healthy but some are in critical condition. Nevertheless, it is important to understand how side-effect is happening among elderlies in general.
以下の例を発見した。I found below cases.
１）100歳近い40kgぐらいの、車いす介護下の女性が9mgを服用後、１週間程度意識不明Near 100yrs old woman around 40kg on wheel chair at nursing home got 9mg and became unconscious for 1wk.２）高齢の疥癬患者16例（平均年齢82.8歳）にイベルメクチン内服治療を行い、副作用は下痢1例，中毒疹1例で共に一過性で軽症であった．16 elderly scabies patients(average 82.8yrs old) took Ivermectin and one got diarrhea and another got toxic rash, both transient and mild.
３）高齢者施設で疥癬患者74例にイベルメクチン200μg/kg，1週間間隔で2回投与した結果，皮疹の増悪，そう痒の増加などの副作用も認められなかった． Ivermectin was administered to 74 patients with scabies at an elderly care facility at a dose of 200 μg/kg twice a week, and no adverse effects such as worsening of skin rash or increased pruritus were observed.４）44人の高齢者の患者に処方したところ、85歳の一名に肝臓障害が出たが、服用を停止すると回復した。When prescribed to 44 elderly patients, one 85-year-old patient suffered liver damage, but recovered when he stopped taking the drug.
結果、健康な高齢者には問題ない印象を受けた。しかし、５）に頻出するように、体力的に弱っている人や、内臓に問題のある人は、服用はあきらめるか、処方添付文書そのままの用量でなく、微量で反応を見て問題が出れば中止する、用量を下げる、長期間に分割して低用量を服用するなどの工夫が必要だろう。In conclusion, healthy elderlies may not have big problem for taking the Ivermectin. However, as in the list in 5), for those with weak health condition or with problem on visceral function, it looks important either to give up taking it, or to tinker a way of dosing, such as testing for small amount and quit if any problem, reduce dosage, or extend the period of dosing for same amount.
