真夏の夜の悪夢
A Midsummer Nightmare
コロナという共通の災厄に立ち向かう、地上で唯一の知的生命体である人類は、コロナに対峙する代わりに、お互いの争いに時間を費やしているなんてありえないだろう。It is impossible that sole intelligent creature on earth, Human Kind, wastes time on internal fight facing the common enemy Covid-19, instead of confronting virus itself.
以下は私のただの妄想の悪夢の物語である。聞き流してほしい。Below is just my obsession, so pls just let it go.
ある既存薬が感染初期から適用外処方するとコロナに有効であろうことが、年初に各地で確認された。その後も、多くの医師がその薬の有効性に気が付き、多くの成功例が発表された。この薬を利用すれば、ワクチンを待たずして、コロナの深刻度は普通の風邪レベルになり、コロナ禍は実質的に終焉するはずだ。Early this year, one generic drug was found to be effective against Covid-19 on off-label basis. Later, many doctors realized its efficacy and many success stories were announced. If the medicine is used, without waiting for vaccine, Covid-19 will be just like normal flue, the current tragedy will disappear substantially.
医師たちは時の指導者にその効果を知らせた。指導者は、これがコロナ禍解決の切り札になると確信し、その利用を国民に薦めた。Doctors informed the leader the efficacy of the medicine. Leader was convinced and promoted the drug to people.
ところが、その指導者と対立する勢力及び、コロナが簡単に終息しては儲からない勢力は、様々な手段でこれを妨害した。あろうことか、その薬が無効であることを証明することで、その薬を薦める指導者を蹴落とそうと考えたのであった。実は、指導者は国を二分する大闘争の下で選ばれ、敗北した反対派は手段を問わず選挙で勝ちたいと思っていたのだ。However, those who has been confronting against the leader as well as pharmaceutical company that cannot make money if Covid-19 ends soon, got together and blocked the widespread of the cure in many ways. Forbidden approach was taken, i.e., they planned to disgrace the leader by "proving" that the medicine does not work. As a matter of fact, the leader was chosen through fierce election process dividing country into two, hence lost-side desperately wanted to win the next election by taking any action possible.
まず、偽の治験データを作成することで薬の無効研究結果を示し、驚いたことに世界・国家の健康機関もこれに倣った。多くの報道機関も薬の無効を喧伝した。ところが、治験データが１万人を超える大規模なものであったことから偽データであったことがすぐに露呈し、無効研究結果は撤回され、データを捏造した人々も姿をくらました。1st, they forged the trial data and proved non efficacy however, surprisingly world as well as national health organization followed the result. Many main-stream media broadcast-ed the non efficacy of the drug. However, due to the over 10K massive number of trial data used, the forgery was soon revealed and research was retracted. Those who made up forged data were disappeared.
また、２つの治験では異常に過大な用量の処方で死亡率を上げることで、薬の無効を証明しようとした。しかし、用量が高死亡率の原因であることは直ぐに皆が知ることとなった。Also in another two clinical trials, they tried to prove the non efficacy by prescribing massive over-dose to increase death rate. However, soon all learned that the result of high death rate comes from massive overdose.
さらに、その薬は外来において、感染初期に投与することで重症化および入院を防ぐのが効能であるのに、重症化して入院してからの患者に処方し、無効であることを主張する研究も相次いだ。その薬は、基本外来ベースでの処方のため、管理された入院ベースの場合と異なり、RCTに乗りにくいことが弱点であった。Besides, lost of other clinical trials were done focusing on patients in last stage or in serious conditions to prove non efficacy, although the medicine is know to be effective only when taken from inception of infection to stop getting serious to be hospitalized. The medicine needs outpatient based treatment from infection inception unlike rather easy-to-control inpatient situation, making RCT more difficult to perform and it is a weak point of the medicine.
世界・国家の健康機関は、薬は有効でなく、危険でさえあるという立場を変えることはなかった。悲しいことに、汚染と腐敗はそこまで及んでいたのだった。World and national health organization kept saying that the drug is not effective, even dangerous. Sadly, contamination and/or corruption have been spread to that level.
指導者には専属の医療アドバイザーがいた。しかし、その医療アドバイザーは製薬会社の利権の中枢であり、その薬が有効であると分かってしまうと収益機会を失ってしまうため、その薬にRCTでの有効結果が無いことを理由に、その薬の有効性を頑なに否定した。一方、自らの利権がある製薬会社の薬は手早くRCT治験を済ませ承認させた。価格はある薬の150倍の高額となったが、政府は思い通り在庫をすべて購入した。ただ、その製薬会社の薬の有効性には他の治験では疑問符がついている。The Leader has a medical adviser. However the medical adviser is actually the core of conflict of interest of pharmaceutical company that lose profit taking opportunity if the efficacy of the drug is known. Hence the adviser stubbornly turned down the efficacy of the drug using the non existence of RCT as a reason. Meanwhile the advisor made another drug approved by rush RCT with which he has strong conflict of interest. The price of another drug was 150 times higher but government purchased all inventory as he planned. Now the efficacy of the another drug is in question by further clinical trial.
しかし国民は黙っていなかった。特に、自分の患者がその薬で殆ど全て回復しているのに、その利用を妨げられた医師たちは、団結し、その薬の開放を主張した。その決起集会の様子はインターネットに流され、指導者もそれを支持した。ところが、２千万人近くの人たちがこの様子を見た直後に、インターネット上から、映像は消失した。インターネットの管理者は映像検閲の結果、その薬が実は、世界・国家の健康機関や指導者のアドバイザーによると有害であるとして、アクセスをブロックしたのだ。But citizens could not keep being quiet. Especially the doctors, whose patients were almost completely recovered by the medicine, got together and stressed the need to open up the use of the drug. The rally they organized was broadcast-ed through internet and the national leader supported it through internet. However, right after 20mn people viewed it, the internet information were deleted. Internet managers censored them based on the view from world and national health organization concluding that it is a false information not to be spread over to public since it is harmful, hence blocked the information.
指導者は直ちにインターネットの検閲を違法とする命令を出した。ある地域では、朝にその薬の利用を禁止したものの、指導者の検閲違法命令が出た同日に、利用を再度許可するというドタバタ劇までついた。The leader immediately ordered to stop internet censorship. One region showed slapstick drama by issuing ban order of the medicine in the morning but took back the order in the evening right after the national leader banned censorship.
さあ、国民は最終的にその薬を手にして、普通の生活を取り戻すことができるのだろうか？Now the question is, if people can get back their normal life by having the medicine in hand.
というところで、目が覚めた。悪夢はまだまだ続きそうだ。Then I woke up from nightmare all of a sudden. But I am afraid that it may continue over and over...
