コロナが欧米で感染爆発するにつれ、世界の時空は捻じ曲がった様相を呈している。米国危機管理チームの日次記者会見はますます緊迫度を増してきている。ニューヨークでは病院がパンク状態だ。トランプは政府支出を１兆ドルから２兆ドルに増額するが、それでも市場は下落を止めない。イタリアでは毎日６００人以上が亡くなる悲惨な状況だ。As Corona infection explodes in US and Europe, time space of world started distorting. US state or emergency team daily update has been getting more and more serious every day. NY's hospital is packed by patients. Trump increased government aids from $1tr to $2tr but market did not stop falling down. Italy is in tragic situation where over 600 patients die every day.
日本はというと、人の国内移動の制限はなく、検査能力は最低限で、症状が出た人も、検査をしてもらえない状態が続いている。基本、検査陽性でも治療方法がないので、各自、安静にして自然治癒してくれということだ。また人工呼吸器もほとんど期待できない状態のようだ。１カ月も前から有効性が指摘されたアビガンは、胎児への催奇性から、すぐに配備されない。在庫が２百万人分もあるというのに。For Japan, there is no restriction on domestic transportation, testing capacity is minimal, hence even if you have symptoms, no testing is done. Bottom line logic is, even if you are tested as positive, since there is no remedy, patients only have option to stay home and wait for natural recovery. Also, cannot expect ventilator as available. Avigan is still not come into front due to teratogenic problem although its effectiveness was pointed out almost one month back with storage for 2million people.
高齢者があふれる日本がイタリア化することはないのだろうか？イタリアに行った中国医療チームが交通遮断を徹底するように指導しているシーンを見た。現状で、日本とイタリアで大きな違いがあるとは思えない。Is there any chance that Japan will be just like Italy? I saw Chinese medical team is advising Italian team to shut down traffic. At this moment, I do not see big difference between Italy and Japan.
医療関係者の中でも、コロナはちょっとひどいインフルエンザ程度であるとの認識を持っている人も多い。また、学校は４月から再開する予定だ。現状、日本の最後の砦は、もともとの衛生レベルの高さだけだ。Even among the doctors, some feel that Corona is just a bit serious influenza. Also, schools will start from April, new semester. Currently the last resort for Japan is only inherent sense of hygiene.
世界の共通の対応方法は、感染スピードを緩和し、患者の発生を医療機関の対応能力以内にコントロールすることだ。日本の対応モデルは、大規模検査により医療機関を大混雑にする代わりに、医療機関が処理能力以上のコロナ検査をしない、というものだ。では認定されない患者はどうなるか？認定されないので、コロナ感染者にはカウントされない。また、もし不幸にして亡くなった場合も、コロナ死者数にはカウントされない。このモデルの問題は明らかで、正確な把握ができないだけでなく、認定されない隠れコロナ患者がさらに感染を拡大させていく。Global common approach is the flattening of infection expansion curve to control patient number under capacity of medical institutions. Japan's model is, however, is to keep number of Corona test under capacity of medical institutions so that hospitals will not be over crowded by having collective test. What will happen to patients who had no luck to get test to be endorsed as Corona patients? They are not counted as Corona case since they are not tested. Even if they pass away unfortunately, they are not counted as death by Corona. The obvious problem of this approach is not just lack of precise statistics on the number of cases but also the expansion of infection by those who did not get test.
ただ、悲しいが、現状の日本にこれ以上の対応は無理だろう。あとは、アビガンが量産され、日本で感染爆発がおこらないことを心から祈るだけだ。However, it is pathetic to say that current way is the best Japan can do at this moment. I only pray that Avigan is mass produced and infection explosion will not happen in Japan.
